Provider Toolkit
No more searching for forms or trying to create your own! The Provider Toolkit contains forms you need to maintain your required provider documentation. All forms are approved for use by the Ohio Department of Medicaid (ODM). Every provider is responsible for maintaining documentation that complies with Medicaid regulations and searching for or creating compliant forms can be a time consuming and difficult barrier for many providers. We hope that you find this resource to be valuable!

If you know anyone who may be qualified and interested in pursuing a career as an in-home service provider, please fill out our provider referral form below. Our recruitment team will be in touch with the referred individual to answer any questions they may have and help with next steps.
As a reminder, qualified individuals must be 18 years of age or older, pass a criminal background check, and be able to read, write and understand English at a level that enables them to comply with program requirements.
Please note that referring a provider does not guarantee their enrollment as an in-home service provider. Also, you will not receive additional compensation based on your referral, nor will your employment be affected based on this individual’s performance as an in-home service provider.
