PCG Match

PCG is committed to supporting and growing the in-home service provider workforce through dedicated education and resources. We developed PCG Match, an online provider-individual matching platform that helps providers connect with individuals near them who are looking for support.

There is a growing gap between the number of individuals who need Medicaid Home and Community-Based Services (HCBS) and the number of providers available to meet this need. We know it can be challenging to safely find individuals to serve, so PCG Match was created to make it easier for you to connect with individuals near you. This enables individuals to remain in their homes and continue leading independent lives.

This system is designed to be used by:

  • Individuals receiving Ohio Home Care and MyCare Ohio Waiver services and/or Medicaid State Plan services. 

  • Independent Providers who are Personal Care Aides, Home Care Attendants, or Nurses.

  • Medicare Certified or Otherwise Accredited Home Health Agency Providers

  • Case Managers supporting individuals on the Ohio Home Care or MyCare Ohio Waivers.

How PCG Match Works

PCG Match is a secure, web-based platform designed to help connect individuals receiving Ohio Home Care or MyCare Ohio Waiver services and/or Medicaid State Plan services with providers who may be able to support individuals' service needs. PCG Match does not replace any existing processes used to vet and/or authorize providers prior to rendering services.

  • Individuals receiving Ohio Home Care or MyCare Ohio Waiver services and/or Medicaid State Plan services and case managers who support them can easily post jobs and use the online provider search to find care.

  • Independent and Agency Providers can search job posts to locate individuals looking for services near them.

  • All users can communicate securely within the system to determine if they are a good match for one another.

PCG Match is a service provided on behalf of the Ohio Department of Medicaid (ODM). PCG Match does not replace any existing processes used to evaluate and/or authorize medical providers prior to rendering services. Only the data PCG Match users choose to share through PCG Match, including personal information (PI) and personal health information (PHI), will be made available to view by other authorized PCG Match system users.
