Provider Enrollment

PCG is responsible for ensuring that waiver provider enrollment requirements are met and that provider applications are completed in a timely and compliant manner. We understand that provider enrollment is the first step toward a successful career and are here to assist applicants with support and assistance throughout the enrollment process.


In addition to application assistance and support, PCG also processes provider applications and verifies that the provider applicant is compliant with the provider qualifications and service requirements outlined in the Ohio Administrative Code. See the following resources that are available to assist you in completing your enrollment application. We are excited to release our Provider Enrollment Success Training Series! The Provider Enrollment Success series contains training videos and guides designed to help providers successfully complete their enrollment application and begin delivering services as quickly as possible.
