PCG is responsible for ensuring that provider applicants meet waiver enrollment requirements and that provider applications are completed in a timely and compliant manner. PCG processes provider applications, verifies that applicants are compliant with the provider qualifications and service requirements outlined in the Ohio Administrative Code, and offers support to providers throughout the enrollment process.

Enrollment Training for Non-Agency (Independent) Providers
The Provider Enrollment Success Training for Independent Providers contains training videos and guides designed to help providers successfully complete their enrollment application and begin delivering services as quickly as possible. This training offers an overview of the application and training requirements for independent Personal Care Aide, Home Care Attendant, and Nurse providers. Helpful tips related to the application process are also included.

Provider Training and Licensure
All applicants must submit appropriate documentation verifying that they meet specific training and licensure requirements set forth in the Ohio Administrative Code. See details about provider training and licensure requirements for each provider type below.
Personal Care Aide
Personal care aide services and eligibility requirements are defined in Ohio Administrative Code 5160-46-04. Individuals seeking to apply as a non-agency personal care aide must provide accurate and appropriate documentation to verify that they have successfully completed a qualifying training program, First Aid Training, and CPR Training.

The first aid certification must be from a class that is not solely internet-based and includes hands-on training by a certified first aid instructor and a successful return demonstration. A copy of the current and valid first aid card or certificate must be included with the application.

Non-agency personal care aide applicants must submit documentation verifying the completion of a training program within the last 24 months. The first and last name on your training document must match your provider application.
There are three options to complete this training requirement: A Competency Evaluation Program, The Medicare Competency Evaluation Program for Home Health Aides, or another equivalent training program. Read more about these training options below.
Competency Evaluation Program
Complete a competency evaluation or a training and competency evaluation program approved by the Ohio Department of Health. Examples of acceptable documentation include:
• Submission of a State Tested Nurse Aide (STNA) number in “Good Standing” with the Department of Health. Verify your status here;
• An STNA training program certificate that does not include testing for an STNA number with the Department of Health; OR
• A Home Health Aide Certificate from a training program approved by the Department of Health.
Medicare Competency Evaluation Program for Home Health Aides
A certificate and a skills checklist verifying you have completed 75 hours of training. 42 CFR 484.80 outlines the requirements for this training option here.
Other Equivalent Training Program
Other equivalent training allows you to meet your training requirements by working directly with a provider, individual, individual’s authorized representative, or individual’s family member to complete training and create a training document. The training document you submit must include your first and last name as written on your provider application, the training completed date, the trainer’s name and signature, and these required training areas:
• Personal care aide services as defined in rule 5160-46-04.
• Basic home safety; and
• Universal precautions for the prevention of disease transmission, including hand-washing and proper disposal of bodily waste and medical instruments that are sharp or may produce sharp pieces if broken.
Home Care Attendant
Home Care Attendant services and eligibility requirements are defined in Ohio Administrative Code 5160-44-27. Individuals seeking to apply as a non-agency personal care aide must provide accurate and appropriate documentation to verify that they have successfully completed a qualifying training program, First Aid Training, and CPR Training.

State Teste Nurse Aide (STNA) Certification
Submission of a State Tested Nurse Aide (STNA) number in “Good Standing” with the Department of Health. Verify your status here.
Medicare Competency Evaluation Program

Certified Vocational Program
A certificate and transcript from a certified vocational program in the health care field. The program must have written testing and skills testing by return demonstration.
Written Attestation of Training
A written attestation of training, instruction, and skills testing by return demonstration may be provided and submitted. The training document you submit must include your first and last name as written on your provider application, the date training was completed, and the trainer’s name and signature.

Home Care Attendants must complete and submit the Home Care Attendant Skilled Task Authorization and Home Care Attendant Medication Authorization, which can be found on the Ohio Department of Medicaid’s Website, here.

Nursing (LPN/RN)
Licensed Practical Nurse (LPN) and Registered Nurse (RN) applicants must submit a copy of their current active and unrestricted RN or LPN license with the Ohio Board of Nursing. Eligibility will be verified through a licensure verification on Ohio Board of Nursing website here.
