The Ohio Department of Medicaid (ODM) is partnering with Public Consulting Group (PCG) to improve Home- and Community-Based Services (HCBS) in Ohio. Please consider completing a survey by July 15, 2022, to help us improve HCBS.
We are inviting individuals, providers, and case managers/care coordinators to complete their respective survey, linked below, by July 15, 2022. Surveys will take approximately 15 minutes to complete.

The Ohio Department of Medicaid has provided updated guidance and FAQ’s regarding the latest updates on COVID-19 impacting OHCW, PASSPORT, and MyCare providers.
The Ohio Home Care Waiver Program case managed by CareSource is moving to a new system platform on April 1, 2021. The changes will enhance the individual and provider experience. There will be new individual and provider portals that will look and act different. Create an account by following this link CareSource.com > Login > Provider. Get help with a click to a help link on your computer or a phone call to 1-800-488-0134. The help desk can also assist with obtaining your unique CareSource portal ID for registration and log in.

Beginning February 1, 2015 and ending no later than June 30, 2015; Individuals enrolled on the Transitions Carve Out Waiver will be moving over to the Passport/Choices waivers. Providers who serve these individuals will need go through the Ohio Department of Aging (ODA) certification process. This notice is to inform all providers of services to individuals on an Ohio Department of Medicaid (ODM) home and community-based services waiver of mandatory training on incident management. An "incident" is an alleged, suspected or actual event that is not consistent with the routine care of, and/or service delivery to, an individual enrolled on an ODM waiver.
To complete this requirement, providers have the option to view the training at: http://ohiohcbs.pcgus.com/TrainingMaterials. Agencies also may choose to train their employees in a different manner. Regardless of how providers complete this training, they must document in writing that they have reviewed the rule.
In addition, upon entering into a Medicaid provider agreement, and annually thereafter, all providers must acknowledge in writing they have reviewed Ohio Administrative Code Rule 5160-44-05 Ohio department of Medicaid (ODM)-administered waiver program: incident management system, which can be found at https://codes.ohio.gov/ohio-administrative-code/rule-5160-44-05

Public Consulting Group (PCG) and its PCG division have partnered with the Ohio Department of Medicaid (ODM) to provide Provider Oversight and Incident Management for its Home and Community-Based Services Waiver programs, the Ohio Home Care Waiver. PCG enrolls waiver providers, conducts provider oversight as well as incident investigations in coordination with providers, case management agencies, local agencies, and the many state sister agencies of ODM.
