Provider Enrollment Application & Resources

PCG is responsible for ensuring that provider applicants meet waiver enrollment requirements and that provider applications are completed in a timely and compliant manner. PCG processes provider applications, verifies that applicants are compliant with the provider qualifications and service requirements outlined in the Ohio Administrative Code, and offers support to providers throughout the enrollment process.

Provider Application

All providers must submit their application through the Provider Network Management (PNM) system, formally the MITS system. Click here to begin your application.

Below, find additional information about obtaining your NPI number, completing your Electronic Visit Verification (EVV) Training, and completing your background check.

  • ODM uses the EVV system for many home and community-based services. EVV is an electronic system that verifies when provider visits occur and documents the precise time services begin and end. It ensures that individuals receive their medically necessary services.

    Services included in EVV are:

    • Home health nursing

    • Home health aide

    • Private duty nursing

    • RN assessment

    • Waiver nursing services provided pursuant to the Ohio home care waiver, the PASSPORT program, the IO waiver, or the MyCare waiver

    • Personal care aide services provided pursuant to the Ohio home care waiver, the PASSPORT program, or the MyCare waiver

    • Home care attendant services provided pursuant to the Ohio home care waiver or the MyCare Waiver

    • PASSPORT home care attendant services

    • Homemaker personal care provided pursuant to the level one and IO waivers

    EVV Training*

    EVV training must be completed for your agency or independent provider enrollment application.

    *Note: If you do not have your Ohio Department of Medicaid (ODM) Provider Medicaid ID number, enter ‘9999999’ in the “Provider Medicaid ID #” field on the registration form. When you receive your ID, please update the Medicaid ID # field in your profile so that ODM and Sandata are notified of your EVV training status. If you are a non-agency user, you may enter ‘Non-Agency User’ in the Agency Name required field.

  • You must obtain an Ohio background check that is processed through the Ohio Bureau of Criminal Identification and Investigation (BCI).

    BCI requires the use of the electronic WebCheck fingerprinting system. A list of WebCheck Vendors can be found on the Ohio Attorney General’s website.

    You must inform the WebCheck vendor that your background check results be sent directly from BCI to ODM at the address below:
